Acute hospitals

Community teams are changing the way they are working to support fast discharges from the acute, and a simple way to refer in to all services.
The community services are forming an integrated team, which will be aligned to Primary Care Network areas. Initially, the scheduling and prioritising of people’s needs will be based in a community coordination centre.

We are introducing a single way of referring into community services, this will help us prioritise people’s needs.

If you are a member of the public and need help you should call 111, contact your GP or the council’s access service.
