We have taken the decision to pause the Embrace Care programme as a whole so we can re-deploy people to support the COVID-19 emergency. As soon as a decision is made to restart the programme we will be in touch. You can sign up to receive the Embrace newsletter here
At the moment, lots of older people go into hospitals or care homes when they would prefer to live independently in their own homes. Through Embrace Care our goal is to develop individual packages of care to enable this to happen. Last summer, we analysed and reviewed 265 cases to check whether the outcomes for the patient were ideal. In around half of the cases, we found they weren’t.
This is mainly because we don’t have connected working practises in place to enable a person to stay safely at home. There are many services available but they are not joined up or people don’t know that they exist. This leads to frustrations for patients and our workforce.
To address this, we are now working with our colleagues to find out what needs to change and how to make that happen.
