Embrace Care

Embrace Care aims to improve the way we care for and support older people in Cornwall and the Isles of Scilly. We want to do this by getting colleagues from all areas of health and care services to work more closely together.

At the moment, lots of older people go into hospitals or care homes when they would prefer to live independently in their own homes. Through Embrace Care our goal is to develop individual packages of care to enable this to happen. How do we know we’re doing the right thing
Last summer, we analysed and reviewed 265 cases to check whether the outcomes for the patient were ideal. In around half of the cases, we found they weren’t.

This is mainly because we don’t have connected working practises in place to enable a person to stay safely at home. There are many services available but they are not joined up or people don’t know that they exist. This leads to frustrations for patients and our workforce.

To address this, we are now working with our colleagues to find out what needs to change and how to make that happen.
