Cornwall has been chosen as a pilot site with funding provided from NHS England. Learnings from the work achieved in Cornwall over the coming months will shape the method of delivery for a national roll out.
The community intervention offer model in the Embrace programme aligns with Ageing Well urgent community response requirements and the pilot phase will run in conjunction to the restart of Embrace.
