Reshaping adult social care

As part of Cornwall and the Isles of Scilly Health and Care Partnership, adult social care is re-shaping their operational teams to mirror as much as possible the locality footprint of health teams in the community integrated areas.

East and West will run a test and learn project with these locality teams for the next three months to test out locality based working. These teams are based on GP clusters and will enable teams to develop closer relationships with their health colleagues and local communities

East and West localities went live on Monday 4 February, with the mid locality going live in the next couple of months as it uses the Kemeneth programme, which is a strengths based approach to supporting people in the community to meet social care needs.

This will mean a dual running of systems for a short while but it is important for Kemeneth approaches to be embedded fully in mid before starting a locality reshape.
